Super Refund Checklist

• Full name
• DOB
• Superannuation fund name (in case you have more than one fund, list all of them)
• Your membership number with all of your superannuation funds (in case there are more than one)
• Your current and all previous addresses while in Australia, and
• The name of the companies where you have worked during your entire stay in Australia.

Once you have gathered all information mentioned previously, these are the documents you will need to get your tax refund process started:

• Copy of all pages of your passport (no blank pages). All copied pages must be certified by a Justice of Peace - JP
• Copy of two valid ID’s - driver’s license, Proof of Age card, student card or any other document with your photo and name
• Other relevant documents to your application, which will be signed and attached to your file
• Once you have permanently left Australia, you will need to mail us a certified copy of your latest custom’s departure stamp
